Our community matrons provide treatment and support to people with long term conditions – such as diabetes, cardiac and respiratory diseases – who have had recent emergency hospital treatment or who frequently consult their GP.
They are a team of senior, experienced nurses with enhanced skills who can prescribe medicines, undertake physical assessments and advanced care planning to help facilitate and promote patient independence without the need for hospital admission.
The team works to improve patient outcomes for people living with long term medical conditions such as asthma, arthritis, diabetes, chronic obstructive pulmonary disease, renal disease and heart failure.
These conditions account for around 80% of GP consultations, and our community matrons work with GPs, the proactive care team, social services, hospital and voluntary services to improve the management of these long-term conditions for people in their own homes.
The team accept referrals from Enfield GPs.
