Community heart failure service 

The community heart failure service works closely with hospital-based cardiology and heart failure teams to help patients manage their heart condition, know when their illness is changing and when to seek help.  

The team provides: 

  • Management of your condition in the community to prevent admission or re-admission to hospital. 

  • A review of your condition within 14 to 28 days after discharge from hospital following an admission due to heart failure at our community clinic at Lucas House, at home or via a telephone consultation depending on your needs. 

  • Review of your heart failure medication to ensure you are on the best tolerated types and doses to ensure it is the most effective it can be. 

  • Support with advanced care planning and liaison with palliative care teams for patients with end-stage heart failure. 

  • Advice and education to help you and your family or carers understand how to manage your condition, identify when it is changing and how to seek help.  

  • Referrals back to your GP for routine care once your condition has stabilised. 

  • Training for healthcare providers about heart failure where necessary. 

The team has clinical support and guidance from cardiologists at North Middlesex University Hospital, the Royal Free Hospital, Barnet Hospital, Chase Farm Hospital, St Bartholomew's Hospital and University College Hospital.  

They also work in partnership with cardiac rehabilitation nurse specialists, pharmacists, palliative care, voluntary care sector teams, physiotherapists and occupational therapists, mental health professionals and social workers across the region.

The service accepts referrals from healthcare professionals for patients registered with a GP in north central London.  

Patients can also self-refer by calling 0121 289 7499. This includes patients who have been discharged from our service but feel they need to be seen again.
