Our stroke rehabilitation service provide care to people living in Enfield who have recently had a stroke and have been sent home from hospital. 

Our team consists of physiotherapists, occupational therapists, speech and language therapists, rehabilitation assistants and a neuro psychologist who work closely with colleagues from the voluntary sector to help you adjust to life at home following a stroke.  

They support you to achieve a good quality of life, maximise your independence and wellbeing and reduce long-term disability. 

Where we’ll see you 

We care for patients within the community — at your own home, workplace, nursing home or residential care. 

This service is for Enfield residents who are aged over 18, registered with a GP, and who have recently had a stroke and are being discharged from inpatient stroke services.

The team accept referrals from: 

  • hyper-acute stroke units  

  • local acute stroke units 

  • other secondary care inpatient rehabilitation providers 

  • self or carer referrals for those who have previously been under our care 

  • GPs 

  • health or social care professionals
