Our stroke rehabilitation service provide care to people living in Enfield who have recently had a stroke and have been sent home from hospital.
Our team consists of physiotherapists, occupational therapists, speech and language therapists, rehabilitation assistants and a neuro psychologist who work closely with colleagues from the voluntary sector to help you adjust to life at home following a stroke.
They support you to achieve a good quality of life, maximise your independence and wellbeing and reduce long-term disability.
Where we’ll see you
We care for patients within the community — at your own home, workplace, nursing home or residential care.
This service is for Enfield residents who are aged over 18, registered with a GP, and who have recently had a stroke and are being discharged from inpatient stroke services.
The team accept referrals from:

hyper-acute stroke units

local acute stroke units

other secondary care inpatient rehabilitation providers

self or carer referrals for those who have previously been under our care

GPs

health or social care professionals
