Our small community nurse-led service offers specialist assessment and advice to patients with non-healing wounds who live in Enfield or are registered with a GP in Enfield.
Patients are seen at home or in nursing homes, at our community clinic at Forest Primary Care Centre, at some of the inpatient mental health services at Chase Farm Hospital, the Magnolia unit at St Michael’s Primary Care Centre and the eating disorder service at St Ann’s Hospital.
Referrals are accepted from healthcare professionals, social services and the voluntary sector via email or post.

They have a wound that shows no signs of healing for four to six weeks after standard care has been implemented for that condition.

Documentation of past and present care and investigations are provided at the time of referral.

The healthcare professional and their team have reached the limit of their tissue viability knowledge.

Referrers keep the patient on their caseload while advice from the tissue viability service is provided.
