Our team help patients with complex type 2 diabetes to manage their condition as well as providing education, support and advice to people with diabetes, their carers and other health professionals.  

The team is led by a diabetes specialist nurse lead, and includes the following diabetes professionals — specialist nurses, health and wellbeing coach, podiatrist and dietician. 

What the service offers 

  • Type 2 diabetes education via the MyDesmond App and virtual and in-person appointments.

  • Consultations for complex diabetes patients including a medication review.

  • Support to empower patients to self-manage their type 2 diabetes and advice for carers and healthcare professionals.

  • Health and wellbeing coach sessions for lifestyle changes.

  • Podiatry care for at risk foot complications. 

  • Community engagement sessions to create awareness about diabetes including free diabetes screening.

Once your referral form has been received, the team will decide which is the most appropriate clinic or clinics for you to attend.  

You may receive appointments for more than one clinic, eg diabetes specialist nurse, diabetes education, podiatrist, health and wellbeing coach and dietician clinic appointments.
